Steps to Become a SOURCE Member

To qualify for our program, individuals must meet specific criteria. Eligibility requires being enrolled in Medicaid and either being 65 years or older or meeting certain disability requirements. Additionally, recipients must require nursing home-level care.

When applying for our program, gathering the necessary documentation is essential. This includes verification of income, assets such as bank accounts or property, and proof of citizenship.

Getting started is easy.  The Medicaid Waiver Program SOURCE is administered through the Georgia Division of Aging Services and Area Agencies on Aging.

To begin the application process, simply call your local Area Agency on Aging (AAA) at the toll-free, state-wide number:  888-669-7195.  An AAA staff member will assist you in screening your situation and guiding you through the initial steps.

Once approved, a Myland Homecare team member will conduct an assessment to identify your specific needs. Based on this assessment, a personalized plan of care will be developed to ensure you receive the appropriate services tailored to your requirements.
