Myland Homecare Behavioral Support Services are provided to eligible clients and require prior authorization based on medical necessity. Our Service is designed to support individuals with challenging behaviors through a structured approach, including:

  • Comprehensive Functional Behavioral Assessment: This assessment involves direct observation, data collection, analysis, graphing, and the development of a behavior support plan.
  • Positive Behavior Support Plan: Based on the assessment results, a tailored Positive Behavior Support Plan is created to address specific behavioral goals and objectives.
  • Ongoing Review and Adjustment: The behavior support plan undergoes routine review to assess its efficacy, and adjustments are made as needed to ensure effectiveness.

Services are provided in various settings, including residential or community settings such as homes, Myland Homecare offices, outpatient clinics, or vocational work sites.

Who Can Apply for Services:

Providers, hospitals, physicians caring for medically fragile children, social service departments, and parents/caregivers of children diagnosed with behavioral disorders can apply for services directly through Myland Homecare.

  • Providers and Hospitals: Physicians caring for medically fragile children can apply during the discharge planning process or at the time of admission.
  • Social Service Department: Social service departments can facilitate applications for individuals in need of behavioral support services.
  • Parents/Caregivers: Parents or caregivers of behaviorally challenged children can apply directly to Myland Homecare by contacting our team.

How to Apply:

To apply for services through Myland Homecare, simply contact our team today. GAPP applications for services may take up to 30 days to review documents once a complete application packet has been submitted to the Alliant Health Solutions Medical Review Team.
