What is CCSP?

The Community Care Services Program (CCSP) is a Medicaid home and community-based waiver services program administered by the Georgia Department of Community Health’s Division of Medicaid in partnership with Georgia’s 12 Area Agencies on Aging (AAAs). This program aims to provide an alternative to nursing home placement by offering tailored services to eligible individuals in their homes and communities.

Eligibility Criteria:

To qualify for CCSP services, individuals must meet specific eligibility criteria, including:

  • Functional impairment due to physical limitations
  • Physician approval for intermediate-level of nursing home care
  • Medicaid eligibility based on financial screening
  • Willingness to receive services at home or in the community rather than a nursing home
  • Participation in one waiver program at a time

Steps for CCSP Eligibility Screening:

  1. Contact Your Area Agency on Aging (AAA): If you’re interested in CCSP services, reach out to your local AAA for an assessment. You can call the toll-free, statewide number at 866-55-AGING (866-552-4464).
  2. Screening and Financial Eligibility: After assessment, if you require a nursing home level of care and meet financial eligibility for Medicaid long-term care, you’ll be placed on a waiting list based on your level of need.
  3. In-Home Assessment and Care Plan Development: Once your name reaches the top of the waiting list, the CCSP care coordination agency in your area will schedule an in-home assessment to determine your care needs and develop a personalized care plan with you and your caregiver.
  4. Service Initiation: After confirmation of your need for nursing home-level care by your physician, the CCSP care coordinator will arrange the start of services outlined in your care plan, which may include care in your home, personal care homes, or adult day health centers.
