Access to our RN Case Managers is readily available through our communication platform, Aaniie. As you contribute to our mission, our Administrator oversees daily operations, and our Executive Administrative Assistant is here to guide you when the Administrator is unavailable.

RN Case Manager – Your Key to Guiding Quality Home Care in Georgia

In this pivotal role, you will provide crucial medical oversight to ensure the effective and safe delivery of care to our clients within the community. Your expertise will be essential in assessing and monitoring each member’s condition, implementing interventions, and preventing unnecessary institutional placements, ultimately enhancing our clients’ quality of life.

As a Case Manager at Myland, you will play a critical role in assigning specific tasks to both unlicensed and licensed staff. Utilizing the “RN Assignment Decision Tree” approved by the Georgia Board of Nursing, you will assess and assign client care tasks based on individual client needs, prioritizing safety and efficiency. Your responsibilities will also encompass various aspects of care, including:

  1. Monitoring and supervising the care provided to members receiving services under a valid PA for GAPP services.
  2. Reviewing referrals, assessing member’s skilled needs, and assigning appropriate staff to meet those needs.
  3. Developing and revising member care plans, ensuring they align with evolving requirements and communicating revisions to the team.
  4. Supervising personal care support personnel, with LPNs under your guidance. You will co-sign and date LPN supervisory visits within 10 business days.
  5. Conducting supervisory assessments, including observations of each Personal Care Support caregiver or LPN providing GAPP member care.
  6. Arranging orientation for Personal Care Support personnel and LPNs for their respective GAPP members.
  7. Completing monthly supervisory assessments to maintain the highest standards of care.
